    People with mental illness are identified as at particular risk of suicide in The Living Is For Everyone: a framework for prevention of suicide and self-harm in Australia (The LIFE Framework). Participants at the 2004 Annual Planning Forum, convened by the National Advisory Council on Suicide Prevention, agreed that a heightened risk existed at particular transition points in the system of care for people with a mental illness, and that the period after discharge from inpatient care was one transition that was particularly critical.
