CAN - Community Connections
CAN - Community Connections provides a hospital to home service and a national non-crisis, peer support telephone line. CAN - Community Connections is managed by Consumer Activity Network (Mental Health) Inc (CAN Mental Health).

Location
- Hospital to Home service is available to people living in the Liverpool, Fairfield and Campbelltown LGA (local government areas) of Sydney South West Area Health Service.
- Phone Connections is a national service available to mental health consumers.
Model
CAN’s philosophy emphasises the value of consumer led service delivery no matter what the consumer's mental health problem(s) may be. CAN - Community Connections project provides two services:
- The Phone Connections service is a national non-crisis, peer support telephone ‘warm line’ that accepts self-referrals by mental health consumers. The uniqueness of Phone Connections is that it does not rely on callers ringing in to receive support. Rather, the peer support workers call consumers after consent is given to provide peer support. The service is currently operating four evenings per week: Monday and Thursday 7pm – 11pm; and Friday and Saturday 6pm – 1.30am.
- The Hospital to Home service is a service that provides practical assistance and peer support for consumers in the 28 days post-discharge from hospital. The primary objective of the service is reducing the hospital re-admission rates of people with a mental illness. Consumers are either self-referred or referred by staff at the Area Mental Health Service to the Hospital to Home service.
Evaluation
The program has a set of evaluation tools and uses an external evaluator to report to the Department of Health and Ageing.
